Player Medical Information Sheet
Hi Team,
Please fill out your Player's Medical Information Sheet as soon as possible. THIS IS IMPORTANT! The Coach/Manager will keep a hard copy of these forms in a binder at every game and practice. If a player needs medical attention, the form will be passed on to the medical personnel treating your player. Some of our players are driving themselves to the rink and a parent may not be present to give this important information.

Hand it in to me or Coach Kevin as soon as possible. I will be at the rink this Saturday, October 17, for our teams RHL Development Camp from 3-3:45 pm and will collect the forms when players arrive and exit the dressing room. Please ask you player to watch for me in the hallway:)
A note about the Covid questionnaire: It can only be accessed for a window of time that begins 24 hours prior to the start of practice, until 1 min prior to the start of practice. Once the practice start time has passed, the form cannot be filled in and the player cannot go on the ice. Thanks for your efforts to fill out the questionnaires in a timely manner and keep each other safe!
